Path to True Success Scholarship Application

THE FOLLOWING MUST BE ATTACHED TO YOUR COMPLETED APPLICATION:

  • A 500-1,000-word essay discussing your knowledge of Federally Qualified Health Centers(FQHC’s) and how your career will impact the community.
  • Two (2) letters of reference from nonfamily member. High school students require one letter of reference from a counselor or instructor.
  • Documentation on organizational letterhead of 75 hours of community service, 20 hours with True Health is preferred. Employees must complete 75 hours of community service outside of True Health. Service must be completed within two calendar years prior to application date.
  • Resume
  • Acceptance letter to vocational program, community college, or 4-year institution.

The 2024 application cycle for the Path to True Success Scholarship has closed. 2024 scholarship recipients will be notified by email.

"My name is Zanisha Premilien, and I'm honored to be one of this year's recipients of the True Health Path to True Success Scholarship. Thanks to your donation, I can continue my education at Valencia College. When I attended my first day of nursing, I was absolutely unsure of how I was going to afford to get through the rest of the four-semester program. I held back tears in my financial advisor's office as I tried to weigh my options for paying for school. The email confirming that I had been awarded the True Health Path to True Success Scholarship was one of the most unexpected things that have ever happened to me. I was so overwhelmed with joy and disbelief that I might have read that email about 50 times. This scholarship is one of the most significant things that could have happened to me. It will allow me to focus solely on school now without the worry about how I will continue to pay for my education."
