Please read before pre-registering!
True Health is currently pre-registering individuals for the COVID-19 Moderna Vaccine at select locations. As of 1/6/2021, the vaccine is only being administered to the patient population identified by the Governor’s Emergency Order. This includes healthcare workers with direct patient contact, individuals 65 years of age and older, and longer-term care facility residents and staff. If you do not fall in one of the above categories, you are not eligible to receive the vaccine at this time. The COVID-19 vaccine will be given at no cost. If you are insured, please bring your insurance card; however, you will not have a co-pay. You do NOT have to be a True Health patient, as the vaccine is open to all eligible community members.
What to expect?
After you pre-register, expect to receive a call from a friendly True Health staff member who will review the information submitted and schedule your initial appointment for the vaccine. If you receive the vaccine at True Health, you will need to return to True Health in 28 days for your second dose. Before leaving your appointment, a True Health staff member will assist in getting you scheduled for your second dose.
